CAMPBELL THERAPY SERVICES, PLLC
Privacy Policy
Effective date: January 01, 2025
Campbell Therapy Services, PLLC respects the sensitivity of seeking mental-health information and services. This Website Privacy Policy explains what information we collect through the Website, why we collect it, how we use and disclose it, and the choices available to you.
This policy applies to the public Website. It does not replace our HIPAA Notice of Privacy Practices, which describes how we handle protected health information subject to HIPAA. When HIPAA or another applicable health-information law provides greater protection, that law and the Notice of Privacy Practices control.
1. Information We Collect
Information you choose to provide
Depending on the Website features you use, you may provide:
your name;
email address and telephone number;
preferred contact method;
general information about the type of service you are seeking;
scheduling or consultation-request information;
insurance information, if requested for eligibility or benefits inquiries; and
other information you choose to include in a message.
Please submit only the information reasonably necessary for us to respond. Website forms and ordinary email should not be used for detailed clinical information or emergencies.
Information collected automatically
When you visit the Website, our website platform and service providers may automatically receive limited technical information, such as:
IP address;
browser and device type;
operating system;
referring page;
pages viewed and approximate visit times; and
information necessary to operate, secure, and troubleshoot the Website.
Some of this information may be collected through cookies or similar technologies, including tools built into our website hosting platform and any scheduling, forms, or analytics tools we use. The categories of cookies and tools actually in use are listed in our cookie notice / cookie banner, which we keep current as our Website configuration changes.
2. How We Use Information
We may use Website information to:
respond to inquiries and consultation requests;
assess service availability and facilitate intake;
communicate about requested services;
verify insurance eligibility or benefits when requested;
operate, secure, maintain, and improve the Website;
understand Website use in a limited or aggregate manner;
prevent fraud, misuse, or security incidents;
comply with legal and professional obligations; and
establish, exercise, or defend legal rights.
Submitting an inquiry does not create a therapist-client relationship.
3. Advertising, Analytics, and Your Choices
We do not sell personal information collected through the Website, and we do not authorize our service providers to use Website inquiry information for their own targeted advertising.
The Website may use analytics or similar tools to help us understand general traffic patterns and Website performance. Depending on how those tools are configured, they may involve cookies or comparable technologies, some of which may be provided by third parties. Our cookie notice / cookie banner describes the specific categories of cookies and tools currently in use on the Website and is updated whenever our configuration changes, so it reflects our actual practices rather than a general statement here.
You can limit or delete cookies through your browser settings. Blocking necessary cookies may affect Website functionality. Where a cookie or tool requires your consent under applicable law, we will provide that choice through the cookie banner before the cookie or tool is active.
Campbell Therapy does not respond to every browser "Do Not Track" signal because no uniform response standard applies. Where legally required and technically supported, we will honor applicable browser-based privacy preference signals.
4. When We Disclose Information
We may disclose Website information only as reasonably necessary:
To service providers that help us host, secure, maintain, or operate the Website; manage forms or communications; or perform another service on our behalf.
At your direction, such as when you ask us to coordinate with an insurer or another person.
For legal or safety reasons, when disclosure is required by law or reasonably necessary to protect rights, safety, or security.
In a business transition, such as a merger, reorganization, or transfer of practice assets, subject to applicable confidentiality and privacy requirements.
We do not authorize service providers to use Website inquiry information for their own targeted advertising. When required by HIPAA, we use a service provider only after an appropriate business associate agreement or other required safeguard is in place.
We do not publish a person's identity, testimonial, review, or clinical information based merely on that person's use of the Website or submission of a form.
5. Health Information and Confidentiality
Information submitted while seeking care may be sensitive and, depending on the circumstances, may be protected by HIPAA, Michigan confidentiality law, or other laws even before treatment begins. We handle such information according to the protections that apply; we do not assume it is unprotected merely because it arrived through a Website inquiry.
If you become a client, our HIPAA Notice of Privacy Practices and clinical consent documents provide additional information about confidentiality, uses and disclosures, your rights, and our legal duties.
6. Security
We use reasonable administrative, technical, and physical safeguards appropriate to the nature of the information we maintain. No website, email system, or internet transmission can be guaranteed completely secure.
If we experience an incident that requires notification under applicable law, we will provide notice in the manner and within the timeframe required by that law.
7. Retention and Deletion
We retain Website information only as long as reasonably necessary for the purposes described in this policy, including responding to inquiries, maintaining appropriate business or clinical records, meeting legal and contractual obligations, resolving disputes, and protecting security.
Retention periods may differ depending on the type of information and whether the person becomes a client. When information is no longer required, we take reasonable steps to delete, destroy, or de-identify it consistent with applicable law and operational needs.
8. Children's Privacy
The Website is intended for adults, including parents and legal guardians seeking services for minors. It is not directed to children under 13, and we do not knowingly solicit personal information directly from children under 13 through the Website.
If you believe a child under 13 submitted personal information directly to us without appropriate permission, contact us so we can review and, when appropriate, delete it.
9. Your Choices and Requests
You may contact us to:
ask what Website information we maintain about you;
request correction of inaccurate information;
request deletion of Website-submitted information; or
ask questions about our privacy practices.
We may need to verify your identity before fulfilling a request. Some information may need to be retained when required for clinical records, legal compliance, security, billing, dispute resolution, or another lawful purpose. Rights concerning protected health information are described in our HIPAA Notice of Privacy Practices.
10. Third-Party Websites
The Website may link to external websites that have their own privacy practices. This policy does not apply after you leave our Website, and we encourage you to review the privacy policy of any third-party service you use.
11. Changes to This Policy
We may update this policy as our Website, practices, or legal obligations change. The effective date above identifies the latest revision.
We will not use previously collected personal information in a materially different way without additional notice or consent when required by law.
