CAMPBELL THERAPY SERVICES, PLLC

Terms of Use

Effective date: January 01, 2025

These Website Terms of Use (the "Terms") apply to your use of the Campbell Therapy Services, PLLC website at www.campbelltherapyco.com and any of its linked pages (collectively, the "Website").

In these Terms, "Campbell Therapy," "we," "us," and "our" mean Campbell Therapy Services, PLLC, a Michigan professional limited liability company. "You" means any person who visits or uses the Website.

By using the Website, you agree to these Terms. If you do not agree, please do not use the Website.

1. General Information Only

The Website provides general information about Campbell Therapy, our clinicians, and our services. Website content is provided for informational and educational purposes only. It is not medical or mental-health advice and is not a substitute for evaluation, diagnosis, treatment, or emergency care from a qualified professional.

Although we work to keep the Website accurate and current, mental-health information changes and may not apply to every person or situation. You should not rely on Website content to make decisions about your health or safety.

2. No Therapist-Client Relationship

Visiting the Website, sending us an email, or submitting an inquiry or consultation request does not create a therapist-client relationship and does not guarantee that Campbell Therapy will be able to provide services.

A therapist-client relationship begins only after Campbell Therapy has confirmed that services are appropriate and available and the required intake and informed-consent documents have been completed.

Clinical services are governed by separate informed-consent, privacy, financial, and practice policies. If those clinical documents conflict with these Website Terms regarding clinical care, the clinical documents control.

3. Not for Emergencies

The Website, email, and website forms are not monitored continuously and must not be used for emergencies or urgent clinical needs.

If you or someone else may be in immediate danger, call 911 or go to the nearest emergency department. In the United States, you may also call or text 988 to reach the Suicide & Crisis Lifeline.

4. Eligibility and Use by Minors

The Website is intended for adults, including parents and legal guardians seeking information about services for a child or adolescent. It is not directed to children under 13, and children under 13 should not submit personal information through the Website.

Michigan law permits some minors to seek or consent to certain mental-health services independently in limited circumstances. If you are under 18, whether you may contact us directly or need a parent or legal guardian to do so on your behalf depends on your age, the services requested, and applicable law. If you are unsure, please have a parent or guardian assist with an inquiry, or contact us directly with questions.

[Drafting note: this paragraph is intentionally general. Have counsel confirm which Michigan consent provisions apply to your specific client population and services, then tighten this language accordingly.]

5. Appropriate Use

You may use the Website only for lawful, personal, and noncommercial purposes. You may not:

  • interfere with the Website's operation or security;

  • attempt to gain unauthorized access to the Website or related systems;

  • introduce malicious code or use automated tools in a way that burdens the Website;

  • impersonate another person or submit information you know is false;

  • use the Website to violate another person's privacy or legal rights; or

  • copy or exploit Website content except as permitted below.

We may restrict access to the Website when reasonably necessary to protect the Website, Campbell Therapy, our clients, or others.

6. Intellectual Property

Unless otherwise identified, the Website and its original content—including text, graphics, branding, photographs, videos, and design—are owned by Campbell Therapy or used with permission and are protected by applicable intellectual-property laws.

You may view, download, or print a reasonable amount of Website content for your own personal, noncommercial use. You may not reproduce, sell, publish, distribute, modify, or create commercial works from Website content without our prior written permission, except as allowed by law.

Campbell Therapy does not claim ownership of third-party names, trademarks, or materials appearing on the Website.

7. Information You Submit

Please provide only information that is reasonably necessary for us to respond to your inquiry. Do not submit detailed clinical information, psychotherapy notes, information about another person without authority, or emergency communications through the Website.

You retain ownership of information you submit. You authorize Campbell Therapy to use that information only as reasonably necessary to respond to your request, evaluate service availability, operate the Website, comply with law, and protect legal rights. These Terms do not give Campbell Therapy permission to publish your identity, clinical information, testimonial, or communications for marketing.

Our handling of personal information is described in the Website Privacy Policy below. If information is protected health information governed by HIPAA or another confidentiality law, the applicable legal requirements and our Notice of Privacy Practices control.

8. Third-Party Websites and Services

The Website may link to third-party websites or services for convenience. Campbell Therapy does not control those services and is not responsible for their content, availability, security, or privacy practices. A link does not necessarily mean that we endorse the third party.

Your use of a third-party service is governed by that provider's own terms and privacy practices. Please review them before submitting information.

9. Website Availability and Disclaimer

We aim to provide a useful and reliable Website, but we cannot promise that it will always be uninterrupted, error-free, secure, or current. The Website and its general content are provided "as is" and "as available" to the extent permitted by law.

Nothing in these Terms excludes any warranty, duty, remedy, or right that cannot lawfully be excluded. This section does not limit Campbell Therapy's responsibilities concerning professional services, confidentiality, protected health information, or data security.

10. Reasonable Limitation of Website Liability

To the fullest extent permitted by law, Campbell Therapy will not be liable for indirect, incidental, special, or consequential losses arising solely from your use of, inability to use, or reliance on the public Website or its general informational content.

This limitation does not apply to clinical services; professional negligence; gross negligence or intentional misconduct; violations of confidentiality, privacy, or data-security obligations; personal injury; or any liability that cannot legally be limited.

11. Changes to the Website or These Terms

We may update the Website and these Terms from time to time. The effective date above will show when the Terms were last revised. If a change materially affects how we handle previously collected personal information, we will provide additional notice or obtain consent when required by law.

Changes apply prospectively when posted unless applicable law requires otherwise.

12. Michigan Law

These Terms are governed by the laws of the State of Michigan, without regard to conflict-of-law principles, and applicable federal law.

Any dispute concerning only the Website or these Terms may be brought in a state or federal court with proper jurisdiction in Michigan. Nothing in this section changes any venue, administrative, licensing, grievance, or other right that cannot legally be changed by agreement.

Before filing a lawsuit concerning only the Website, the parties are encouraged—but not required—to attempt to resolve the concern through good-faith communication. This does not delay or limit either party's right to seek emergency relief, meet a legal deadline, submit a licensing or regulatory complaint, or exercise any nonwaivable right.

13. Severability and No Waiver

If a provision of these Terms is found unenforceable, it will be limited or removed only to the extent necessary, and the remaining provisions will continue in effect.

Our decision not to enforce a provision on one occasion does not waive our right to enforce it later.
